Must Be Created Breakdown Slip
In the event of a malfunction in a machine, a breakdown slip must be created to document the issue and initiate the repair process.
Part A
The slip consists of two parts, Part A and Part B. Part A includes the name and identification number of the machine, the name of the operator, and a description of the problem encountered. It must also be updated with the date and time of the issue. Once completed, Part A is printed out for easy reference when filling out Part B.
Part B
B-Parts are serviced by the maintenance department. The process includes identifying the problem, determining the method of correction, and implementing a solution to prevent future issues.
